- From this page you can order an ID card, download claim forms, find an in-network dentist near you, see your claim history, and see the full terms of your policy.
- To find the full terms of your policy, click on the blue tab near the top of the page entitled “Covered Services”. On the right hand side of this page is a link to your Certificate of Insurance which provides a detailed explanation of benefits.
